Best pillows for back pain
For back pain specifically, the pillow that helps most is frequently not the one under your head.
I earn a commission if you buy through some links on this page. It never changes what I recommend, and I publish negative verdicts on brands I earn from. Editorial independence.
A head pillow's job is the neck. It fills the gap your shoulder creates so your neck stays in line with your spine, and getting it wrong causes neck ache rather than back ache.
Back pain responds to what your pelvis is doing, and the pillows that change that are positional: one between the knees if you sleep on your side, one under the knees if you sleep on your back.
There is a connection at the head end and it is indirect. A pillow at the wrong height tilts your whole upper body, and a chin held toward the chest for eight hours pulls through the upper back. That is worth fixing and it is not usually where lower back pain comes from.

The picks
The pick, if you want one answer
Simba Body Pillow, 7.4 / 10. The positional support that changes what your pelvis does, which is what back pain actually responds to.
| Product | Best for | From | Score |
|---|---|---|---|
| Simba Body Pillow | Best for back pain | Mid range | 7.4 / 10 |
| Simba CustomFlex Pillow | Best head pillow | Mid range | 7.7 / 10 |
| Kally Sleep Body | Best value positional support | Budget to mid | 7.3 / 10 |
| Simba Mattress Topper | If the mattress is the cause | Mid range | 7.5 / 10 |
Simba Body Pillow: Best for back pain
Mid range · 7.4 / 10
Scores 7.4. Lying on your side, an unsupported top leg falls forward and rotates your pelvis while your shoulders stay put, twisting your spine along its length all night. Holding the leg at hip height stops it. Test it free with a folded duvet first.
Buy it if
You sleep on your side and your lower back aches in the morning.
Do not buy it if
You sleep on your back. Use a pillow under the knees instead.

Simba CustomFlex Pillow: Best head pillow
Mid range · 7.7 / 10
Removable fill, adjusted after sleeping on it. Height is what decides whether a head pillow works, and a pillow that lets you change it is worth more than one that guesses well.
Buy it if
You have not established what height your neck needs.
Do not buy it if
You know your height and want it held with no adjustment.

Kally Sleep Body: Best value positional support
Budget to mid · 7.3 / 10
Scores 7.3. Most people discover within a fortnight whether positional support helps them, which makes an inexpensive first purchase sensible.
Buy it if
You want to test the format without spending much.
Do not buy it if
You need maximum firmness under a leg.

Simba Mattress Topper: If the mattress is the cause
Mid range · 7.5 / 10
The highest scoring topper here at 7.5. Back pain from bedding is usually the mattress rather than the pillow, and the direction matters: too firm and too soft produce different symptoms and opposite fixes.
Buy it if
Your hip or shoulder aches side sleeping and the mattress is sound.
Do not buy it if
Your lower back aches on waking. A topper makes that worse.

Which symptom points where
| What you notice | Usually means | Fix |
|---|---|---|
| Lower back ache on waking, eases as you move | Mattress too soft, or pelvic rotation | Knee support, or firmer support beneath |
| Hip or shoulder pain side sleeping | Mattress too firm | A topper |
| Ache at the back of the neck | Pillow too high | Lower pillow. Free. |
| Ache at the front of the neck or jaw | Pillow too low | Higher pillow. Free. |
| Upper back and shoulder tension | Pillow height, or two pillows stacked | One pillow at the right height |
Two stacked pillows is the single commonest self-inflicted cause of upper back and neck tension, because it pushes the head forward and holds it there all night.
The free tests, by position
Side sleepers: a folded duvet between the knees, from knees to ankles, for a week. If it helps, a proper bolster helps more because it stays where you put it.
Back sleepers: a pillow or rolled towel under the knees, which lets the pelvis tilt slightly and takes tension off the lower back. This is the highest-value free change on the page.
Front sleepers: a thin pillow under the hips to reduce the arch in the lower back, and a very low pillow or none under the head. Front sleeping is generally hardest on the lower back, and some people move to side sleeping by leaning into a body pillow, which a folded duvet tests for nothing.
Give each a fortnight. Changing sleeping position is uncomfortable for the first few nights regardless of whether the change is a good one.
Getting the head pillow right anyway
Lie in the position you actually sleep in for ten minutes so the pillow settles, then have someone look at you from behind. Your neck should be a straight continuation of your spine.
Two involuntary signals beat any amount of guessing. An arm habitually under the pillow means too low. Waking having pushed it aside means too high.
And re-run the test whenever the mattress changes, because a softer mattress lets your shoulder sink further and you need less pillow. A pillow that suited you for years can be wrong the week a new bed arrives.
Check the pillow is not simply finished: fold it in half and let go, and if it stays folded it has lost its structure. Foam fails invisibly by losing height while looking identical.
When this is not a bedding question
Most morning back pain that eases within half an hour of getting up is mechanical and responds to the changes above.
Pain that is worse at rest and better with movement, wakes you at night, or has been building for weeks needs a GP or physiotherapist rather than a purchase.
So does any back pain with numbness, weakness, unexplained weight loss, fever, or any change in bladder or bowel control, which needs urgent medical attention.
If a clinician has advised a particular sleeping position or support, take the specifics from them and use this page only for choosing between products.
What not to do
Stack two pillows. The commonest self-inflicted cause of upper back tension.
Buy a head pillow to fix lower back pain. Positional support does more.
Buy anything before the free tests. A folded duvet and a rolled towel answer most of this.
Add a soft topper if your lower back aches on waking. Wrong direction.
Treat contoured or orthopaedic as a specification. Neither word is regulated.
Also considered, and why they are not here
A roundup is only as useful as what it leaves out, so here is what was assessed and rejected for this list.
| Product | Why it is not on the list |
|---|---|
| Simba Hybrid Pillow | The highest scoring pillow on this site at 7.9. Excellent at holding a height, and a head pillow, so it addresses the neck rather than the back. |
| Origin Coolmax Latex | Scores 7.8. Holds a fixed height for years, which is only useful once you know the height you need. |
| Derila | Scores 4.8, the lowest on this site. A small fixed-loft foam pillow sold through urgency mechanics rather than specification, and frequently marketed at pain. |
| Silentnight Airmax Topper | Scores 7.4. If the mattress is too firm at the hip or shoulder, this is the intervention rather than any pillow. |
How this list is made, and who made it
Independence
No brand pays to appear on this site and no placement is sold. Some links earn a commission if you buy, which never changes a score, an order or a verdict. Several of the lowest scores here are on products that carry commission, and several recommendations carry none.
Who writes this. Kyle Klinkradt has worked in the mattress and bedding industry for over ten years across retail, distribution and brand partnerships, and has handled, sold or assessed products from every major UK bed-in-a-box brand. Every review on this site is written by one person, so the scores mean the same thing from page to page.
What the assessment is. Construction specifications against what they physically do: spring height and where in the mattress it sits, spring count for the size, gauge, whether the base is sprung or foam, foam density where it is published, comfort layer depth and material. Then the commercial terms that decide what you are actually buying: trial against returns policy, guarantee length against the company's UK trading history at Companies House, and the regulatory record.
What it is not. This is not a laboratory. I do not publish pressure-mapping data or claim to have slept eight years on every mattress here, because nobody has and pages that imply otherwise are not telling you the truth. Where a judgement rests on construction rather than long-term use, the review says so.
What gets a product removed. Leaving the UK market, a specification change that makes the review inaccurate, or a corrected fact. Brands that have withdrawn are kept on the site with the score removed rather than quietly deleted, because people still search for them.
See editorial independence, corrections and the author page.
How these were chosen
Every product here has its own review on this site with a score out of 10, and the score is a judgement rather than an average. Nothing appears in a list because it pays more.
A score is against the market, not against its own range. The cheapest mattress in a lineup is usually priced to make the model above it look reasonable, and the flagship of a budget range has risen into a bracket where better brands compete. Both are judged against what else you could buy for the money.
Everything listed can actually be bought in the UK. That is not a given: Casper withdrew in July 2020 and Leesa no longer sells here, and both still appear in current British roundups with affiliate links attached.
I do not pad the list. Where only three or four products genuinely qualify, there are three or four. A list of seven when the category contains four good ones is a list with three bad recommendations in it.
Prices move constantly. Judge the cash price on the day rather than any percentage beside it. See every review on this site or where the real savings are.
Questions people ask
What pillow is best for back pain?
For lower back pain, usually not a head pillow. A bolster between the knees if you sleep on your side, or a pillow under the knees if you sleep on your back, changes what your pelvis is doing, which is what back pain responds to.
Can a pillow cause back pain?
Indirectly. A pillow at the wrong height tilts your upper body and a chin held toward the chest for eight hours pulls through the upper back. Two stacked pillows is the commonest self-inflicted cause of that tension.
How do I check my pillow height is right?
Lie in the position you actually sleep in for ten minutes, then have someone look from behind: your neck should be a straight continuation of your spine. An arm habitually under the pillow means too low; waking having pushed it aside means too high.
When should I see a doctor about back pain?
If it is worse at rest and better with movement, wakes you at night, or has been building for weeks. Urgently if there is numbness, weakness, unexplained weight loss, fever, or any change in bladder or bowel control.
