Verdict
Nine out of ten firmness with 4,000 springs, which makes this one of very few genuine orthopaedic-grade hybrids in the UK at this price. If you have been told to sleep on something firm, or you are heavier and tired of mattresses failing at year three, this belongs on your shortlist alongside Otty's Extra Firm.
Buy it if
You need genuine firm support: heavier build, a clinician has advised firm, or you have gone through mattresses quickly.
Avoid it if
You want firm as a preference rather than a requirement. Nine out of ten is severe.

The scores
A serious firm mattress at a fair price, in a segment with almost no competition.
Four thousand springs at 500 pounds in a specialist category is strong.
Four thousand springs at 9 out of 10. This is what heavier sleepers actually need.
A dense spring layer moves air well, and firm builds trap less heat than plush ones.
Firm spring construction holds the perimeter better than softer builds.
The weakest score. A firm sprung mattress transmits more movement.
Standard terms, with broad discount eligibility.
What orthopaedic actually means, and does not
Orthopaedic is not a regulated term in UK mattress retail. Any manufacturer can apply it to any mattress, and many apply it to mattresses that are simply firm.
What it should mean in practice is a support structure designed to hold the spine in neutral alignment under load, which requires two things: enough firmness that the heaviest parts of the body do not sink disproportionately, and enough spring count that different regions can respond independently. Firmness alone gives you a plank. Spring count alone gives you a hammock. You need both.
The 4000 Ortho Elite has both, which is why it earns the description where many do not. Four thousand springs at 9 out of 10 is a genuine combination rather than a label.
If a clinician has advised a firm mattress, take that advice from them rather than from a product name, and bring the specification rather than the marketing to the conversation.
Against the Otty Extra Firm
These two are the main options in the UK for a genuinely firm hybrid, and they are 25 pounds apart.
The Rem-Fit gives you 4,000 springs. Otty's Extra Firm gives you up to 2,000 at a greater height, with reinforced side panels and a longer established UK track record. The Rem-Fit gives finer gradation; the Otty gives taller springs and better edges.
For a heavier sleeper I would lean Otty for the edge reinforcement and the spring height. For someone who wants firm support with more contouring across the surface, the Rem-Fit is the better answer. Neither is a mistake.
A correction worth making
Commonly repeated, and wrong
Orthopaedic is used across UK mattress retail as though it were a specification, and it is not a regulated term. A mattress can be sold as orthopaedic on the basis of firmness alone. Genuine orthopaedic support requires firmness and a high spring count together, because firmness without gradation produces a plank and gradation without firmness produces a hammock. Check the spring count alongside the firmness rating before believing the word.
Should you buy it?
Yes, if you need firm rather than prefer it. This and Otty's Extra Firm are the two UK options I would actually recommend in this category.
If you want firm but are under about fourteen stone, the Otty Original at 7 out of 10 is the sensible level.
Questions people ask
What does orthopaedic mean on a mattress?
Nothing regulated. Any manufacturer can apply it to any mattress and many apply it to anything firm. Genuine orthopaedic support needs firmness and a high spring count together: firmness alone gives you a plank, spring count alone gives you a hammock.
Rem-Fit 4000 Ortho Elite or Otty Extra Firm?
Twenty-five pounds apart. Rem-Fit gives 4,000 springs and finer gradation. Otty gives taller springs, reinforced side panels and a longer UK track record. For a heavier sleeper I lean Otty for the edges; for more surface contouring at firm support, Rem-Fit.
