Not before the cot stage is over
UK safer sleep guidance is that a cot should be clear: no pillows, duvets, cot bumpers, pods or loose bedding. Pillow use starts once a child has moved out of a cot, generally from around 18 months at the earliest, and later is fine.

A pillow fills the gap between the head and the mattress, and that gap is created by the shoulder. A toddler's shoulders are a fraction of an adult's width, so the gap is a fraction of the size. An adult pillow under a small child pushes the head forward and holds the neck in flexion all night.

When a child is ready
There is no age that applies to every child, and the practical signals are consistent.
Out of a cot and in a bed. That is the threshold, because cot guidance is about a clear sleeping space rather than about pillows specifically.
Asking for one, or improvising one with a folded blanket or an arm. That is the same involuntary signal adults give: if a child is adding height in their sleep, they need some.
No rush. A child who sleeps well without a pillow does not need one, and going without is not a problem to solve. If you are unsure, your health visitor is the right person to ask.
How to check the height
The same test as for an adult, scaled to a smaller person.
Have them lie in the position they actually sleep in for a few minutes, then look from behind. The neck should be a straight continuation of the spine: head tipped up means too high, dropped toward the mattress means too low.

A folded towel is a perfectly good way to test a height before buying anything.
Buy for washing, because you will
A child's pillow deals with far more than an adult's: illness, spills, nosebleeds, and simply more frequent accidents.
Check the pillow itself is washable, not just the cover. Foam and latex cannot be machine washed. Synthetic fill can, which makes it the sensible fill for a child even though it holds height least well.
If anyone in the house has a dust mite allergy, wash at 60 degrees. Mites survive a 40 degree wash: 40 removes allergen and the population returns.
Buy two. Changing a bed at 3am is considerably easier when the spare is already clean, and the same logic applies to mattress protectors.
And the rest of the bed
A waterproof protector is the highest-value purchase in a child's bedroom. A stain voids most mattress guarantees absolutely, and over the life of a child's mattress a spill is close to certain.
A 7 to 10.5 tog duvet. Children generally sleep warmer than adults and 13.5 tog is more duvet than most British bedrooms need.
A firm mattress. A child barely compresses a mattress, so a high spring count does very little for them and firm is the right specification.
What not to do
Put a pillow in a cot. Safer sleep guidance is that the cot should be clear.
Use an adult pillow. It holds a small neck in flexion all night, and the effect is usually read as the child being unsettled.
Buy a memory foam pillow for a child if washability matters, because foam cannot be machine washed.
Buy a contoured pillow. A moulded curve helps only if its fixed height happens to match, and a growing child's does not stay matched.
Rush it. A child sleeping well without a pillow does not need one.

What the assessment is. Construction specifications against what they physically do: spring height and where in the mattress it sits, spring count for the size, gauge, whether the base is sprung or foam, foam density where it is published, comfort layer depth and material. Then the commercial terms that decide what you are actually buying: trial against returns policy, guarantee length against the company's UK trading history at Companies House, and the regulatory record.
What it is not. This is not a laboratory. I do not publish pressure-mapping data or claim to have slept eight years on every mattress here, because nobody has and pages that imply otherwise are not telling you the truth. Where a judgement rests on construction rather than long-term use, the review says so.
What gets a product removed. Leaving the UK market, a specification change that makes the review inaccurate, or a corrected fact. Brands that have withdrawn are kept on the site with the score removed rather than quietly deleted, because people still search for them.

How these were chosen
Every product here has its own review on this site with a score out of 10, and the score is a judgement rather than an average. Nothing appears in a list because it pays more.
A score is against the market, not against its own range. The cheapest mattress in a lineup is usually priced to make the model above it look reasonable, and the flagship of a budget range has risen into a bracket where better brands compete. Both are judged against what else you could buy for the money.
Everything listed can actually be bought in the UK. That is not a given: Casper withdrew in July 2020 and Leesa no longer sells here, and both still appear in current British roundups with affiliate links attached.
I do not pad the list. Where only three or four products genuinely qualify, there are three or four. A list of seven when the category contains four good ones is a list with three bad recommendations in it.
